Boat Release This is a two page form. Please print it, complete it and bring it with you to the boat on your first day of diving. If you forget we will have blanks on board for you but it will speed things up if you bring it. Medical Statement This is also a two page form. You only need to complete it if you are doing training with us. Page one has a variety of medical questions which must be answered 'yes' or 'no'. If you have any 'yes' answers, you will need to get page two signed by your physician. It is much more convenient for you to accomplish this prior to your travel to Cozumel. If you forget, we can assist you once you arrive. |
